Please note, as the risk of COVID-19 is still at a substantial risk, we ask all patients to take into consideration face coverings and social distancing when in the surgery.

COVID Vaccination Status Scheme

Your vaccination status is a record of the coronavirus (COVID-19) vaccinations you have received. Your status includes your name, date of birth, and any coronavirus vaccinations you have received in Scotland.

Get a record of your coronavirus (Covid -19) vaccination status from NHS Inform or by phoning the COVID-19 Status Helpline on 0808 196 8565. The helpline is open every day from 10:00am to 6:00pm.

  • GPs and their teams cannot issue vaccination exemption certificates  – Telephone the helpline on 808 196 8565 or visit NHS Inform
  • COVID VACCINATION: Please DO NOT  contact the practice to ask for a COVID-19 vaccination. You will be contacted when you are eligible. Further  information about (COVID-19) vaccine
  • Face Mask Exemptions – The Practice will not provide letters/certificates for face mask exemptions
  • Visit NHS Inform for the latest guidance on coronavirus COVID-19, including symptom checker, self-care and social distancing
  • If you have severe COVID-19 symptoms and need further advice call NHS 24 on 111 — not the surgery
  • Isolation Notes are available online from NHS 111, if you are self-isolating and need a note for your employer.

  • Vitamin D for High Risk Patients – Patients originally on the shielding list (now classed as High Risk) can obtain free vitamin D supplements – these are not available from the surgery – if you are registered with the Government texting service, please text VITD to 07860 064525 from your mobile to request your Vitamin D. You can join this texting service by phoning with your CHI (Community Health Index) number (will be on your shielding letter) to 07860 064525 then text VITD to 07860 064525 once you have done this.
